Job Description

Pride Health is hiring Home Health LPNs / LVNs to support our client in Spokane, WA. This is a 14 weeks of assignment (Possibility of extension or conversion). opportunity and a great way to start working with a top-tier healthcare organization!

Job Title : LPN / LVN - Home Health

Location : Spokane, WA

Job Type : 3 months Contract

Schedule : 8 hrs / Day - Day shift

Expected hours : 40 hours in a week

Position Summary :

Description : Primary function is to administer skilled nursing care to patients in their place of residence, as prescribed by the physician and in accordance with the established plan of care. The LPN / LVN works under the supervision of a Registered Nurse (RN) and coordinates care with the interdisciplinary team as well as the patient / family. The LPN / LVN will deliver care in compliance with State, Federal and Medicare standards.

Essential Duties and Responsibilities :

Supports nursing care under RN supervision by observing and reporting on patient needs and outcomes, including physical, psychosocial, and environmental factors.

Participates in implementing and coordinating patient care plans while ensuring effective communication with patients, families, and the healthcare team.

Assists patients and families in learning self-care techniques and provides care using proper infection control practices.

Performs duties related to hospice care, including admissions, discharges, and transfers, and may participate in on-call responsibilities.

Maintains documentation using electronic charting (e.g., Homecare Homebase), adheres to safety and professional boundaries, and completes all required training and compliance activities.

Required Qualifications :

License / Certification :

Current state license as LPN - Required

Current BLS certification - Required

Current driver's license - Required

Must have reliable transportation and insurance - Required

Experience :

Homecare Homebase Software Experience - Required

2 years' experience as an LPN / LVN in a clinical setting, preferably in a home health or hospice setting

Education :

Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing
