Evaluation tools Billing / Documentation Must have home health experience; OASIS training preferred Certs required : BLS state licensed Scrubs : Any solid color scrub Unit Info (share with your traveler) : Patients : med / surg cardiac ortho hips / knees / shoulders post-surgical copd chf pneumonia CVA Falls weakness parkinsons failure to thrive resp patients neuro trachs trauma AKA BKA ADLS Skilled Home Care (Medicare Medicare Advantage and Commercial ins.) Home skilled visits - 6-8 per day (MINIMUM of 6 points per day at managers discretion may increase by 1 point) Laptop MiFi for internet access Portable printers BayCare Forms BayCare Scheduling Ace Timesheet documentation system Other requirements : 1 year experience required 2 preferred (must be independent practitioner) Must have valid drivers license and car to make home visits. Must be COVID vaccinated or with exemption form on file. Must have flu shot or declination and in both cases mandatory mask wearing at all times while at work. Based on the needs of the HomeCare office the visits and hours will be self-scheduled .01
