Job Description

We are hiring a full-time Nurse Practitioner or Physician Assistant to join our Wound Care / Home Health team in Centralia, WA.

• Responsibilities include evaluating and managing acute and chronic wounds, performing dressings, medically necessary debridement, and G-Tube management (training provided).
• Participate in weekly bedside wound rounds and conduct Home Health visits as needed.
• Must be outgoing, energetic, and experienced in skilled nursing facilities.
• Required skills : strong communication, ability to work independently, and at least 2 years of wound care and skilled nursing experience, including debridement.
• Preferred : Wound Care Certification.

Compensation ranges from $125,000 to $180,000 annually, with a day shift, Monday to Friday schedule, on-call duties, and weekend availability.
