POSITION DUTIES - Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans

MINIMUM REQUIRED QUALIFICATIONS - CA RN license

BLS

2 years recent acute hospital care coordination experience

Prior CommonSpirit / Dignity experience

LENGTH OF ASSIGNMENT - 13 weeks

SHIFT / HOURS PER WEEK - 8 hr Days, EOW (40hrs / wk)

SYSTEMS - Cerner

START DATE - 11 / 3 / 2025
