Job Summary

As a Home Health Registered Nurse (RN), you will be responsible for assessing, developing, implementing, and evaluating nursing care for your patients. You will play a crucial role in coordinating care with other disciplines and agencies involved in the patient's care.

Essential Functions

Communicate effectively with other disciplines regarding patient care documentation and agency functions. Engage in supervision discussions and participate in team meetings regularly.

Document identified patient problems, interventions, discharge planning, physician orders, and care plan updates accurately upon admission and throughout treatment.

Maintain open communication with medical staff to ensure coordinated patient care while adhering to the highest standards of professionalism and agency conduct.

Conduct thorough assessments and interventions at scheduled intervals, adjusting care based on changes in patient condition.

Initiate discharge planning in advance of discharge dates to address ongoing patient needs, ensuring patient confidentiality throughout.

Submit all necessary paperwork and documentation in accordance with agency policies. Participate in chart audits and other Quality Improvement initiatives.

Be flexible and accept assignments beyond normal duties to meet agency needs.

Perform additional responsibilities as assigned.

Supervisory / Management Responsibilities

This position reports to a supervisor, manager, director, or executive, and does not have management responsibilities.

Minimum Requirements

Education : Associate degree in Nursing

Experience : Minimum of one (1) year of nursing experience as a Registered Nurse

Required Certifications, Registrations, Licenses

Active Registered Nurse license in South Carolina.

Current Basic Life Support (BLS) certification.

A valid driver's license and a satisfactory motor vehicle record; proof of auto insurance is also required. This position necessitates using your own reliable vehicle for work-related duties, complying with the Prisma Health Driver Safety Policy.
