Key Responsibilities for the Family Medicine Nurse Practitioner :

• Provide compassionate, individualized primary care to patients in an outpatient setting.
• Deliver care both in person and virtually, building and managing a panel of patients.
• Collaborate with colleagues, community partners, and emergency medical / psychiatric service providers to ensure comprehensive care.
• Participate in after-hours on-call rotation 3–4 times per year.
• Efficiently document within an electronic health record and conduct telemedicine visits.
