The Registered Nurse (RN) provides skilled nursing care to home health patients, focusing on wound and skin assessment, particularly for individuals with diverting ostomies. This role involves collaboration with healthcare teams, educating patients and staff, and precepting nursing students. Required qualifications include a current California RN license, a BS degree, and two years of clinical experience, with a preference for those with home health experience and Wound, Ostomy, Continence Nurse Certification.

Home Health / Hospice :

The CWOC RN will administer skilled nursing care to patients at home.  Provides care for individuals who have, or will have, a diverting ostomy.  Provides wound and skin evaluation and care.  This specialized nursing care is given independently and in collaboration with other members of the healthcare team under the direction of and in accordance with the physician’s orders.  Functions as an educator for patients / significant others and staff.  Precepts staff and students in the specialty.
