Scope of Role :

Home health PT – will provide direct patient care in the patient's home environment as part of a multidisciplinary team. The therapist will evaluate patients and use professional judgment to design the individualized plan of care which best meets the patient's needs. The therapist will use guided interventions to improve the safety and functional independence of its clients through the rehab process by providing direct one-on-one patient care and will also supervise care provided by assistants.

Must have at least 1-year Home Health experience ability to use laptop for documentation and cell phone for communication.

Patient Ration : 5-7 points per day number of patients could vary based on visit type such as Start of Care vs. Routine Visit

Shift Hours : Mon-Fri 8a – 4 : 30p 5x8’sWeekend Requirements : As needed but typically every 5th WeekendOn-Call- Requirements : NoneDuration of Assignment : 13-26 weeks
