Salary and Benefits

• Salary : $225,000
• Sign-On Bonus : $20,000
• Relocation Assistance : Up to $5,000
• Quality Incentive Bonus - Up to $4,200 quarterly for meeting all 12 quality measures (pro-rated for part-time)
