Your Role

We’re seeking a compassionate and skilled Home Health Registered Nurse (RN) to join our growing team. In this role, you’ll use your clinical expertise and assessment skills to provide one-on-one care to patients in their homes, ensuring safety, comfort, and optimal recovery.

What You’ll Do

Conduct OASIS assessments and create individualized Plans of Care tailored to each patient’s needs

Perform skilled nursing visits, including wound care, ostomy management, IV infusions, and PICC line care

Educate patients and families on disease management, medications, and home care safety

Collaborate with physicians, therapists, and other team members to ensure coordinated, patient-centered care

Document all visits, interventions, and progress accurately and on time

Monitor patient status and promptly report changes to the appropriate provider

What We’re Looking For

Active Florida Registered Nurse (RN) license (required)

Home Health and OASIS assessment experience (required)

Solid clinical background with strong critical thinking skills

Excellent communication and time management abilities

Current CPR / First Aid certification

Valid driver’s license and reliable transportation
