Hospice : RN Case Manager (NE Metro)

The Hospice RN Case Manager plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education / experience. The Hospice RN Case Manager builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities.

Position : RN Case Manager for Hospice

Wage Range : $88,000-$94,600 annually, depending on experience

Service Area : Northeast metro (coverage area : Roseville, Arden Hills, New Brighton, Shoreview, Mounds View, Circle Pines, Lino Lakes, Forest Lake, Wyoming, Scandia, Hugo, White Bear Lake, Mahtomedi, Stillwater, Lake Elmo, Woodbury, Oakdale, North St. Paul and surrounding). Plus, mileage reimbursement of .70 cents per mile.

Schedule : 1.0 FTE, Monday - Friday 8 : 00am-5 : 00pm (with every 5th weekend rotation and 2 holidays / year)
