Job Responsibilities

We are seeking an experienced Registered Nurse (RN) to join our team as a Home Health Evening Registered Nurse. In this role, you will provide direct patient care to patients with illnesses, injuries, and chronic diseases. Your duties will include :

Conducting thorough assessments of patients' physical and emotional needs

Developing and implementing individualized care plans tailored to each patient's specific needs

Providing education and support to patients and their families on disease management, medication, and therapy

Collaborating with multidisciplinary teams to ensure seamless care coordination

Requirements

To be considered for this rewarding opportunity, you must have :

A graduate degree in nursing from an accredited program

An active and unrestricted RN license in the state of South Carolina or compact multi-state license

A minimum of one year of nursing experience, preferably in home healthcare, med / surg, or critical care
