Travel Physical Therapist (PT) - $2,678 per week in Manchester, NH

Description : The position involves providing physical therapy services in patients' homes. The therapist will be responsible for conducting assessments and creating individualized treatment plans. Visits will be made to approximately 6 patients daily. Patient demographics include adults and geriatric populations with various acute, subacute, and chronic diagnoses. Therapists will provide interventions focused on fall risk reduction, strength training, and gait training. Collaboration with a multidisciplinary team is essential to achieving patient goals. Documentation will be completed using an electronic health record system. Therapists must be skilled in using adaptive devices and equipment as needed for patient care. The role requires time management skills to efficiently manage patient visits. Applicants should have experience in home health or a similar setting. Additional skills such as critical thinking and effective communication are required for this position. Weekend work may be occasionally required depending on patient needs.
