Mercy outpatient rehabilitation team is a leader in evidenced based neurological care. All therapist work closely with the talented physicians here at Mercy Health. The treatment model is impairment based and focused on; a hands on evidence based care that is delivered on a one to one basis. Therapist are trained in the latest advanced skills.

Specifically youll need :

  • To evaluate plan and implement Physical Therapy services using an impairment base approach according to the standards of practice governing laws of the State of Ohio and facility and department policies and procedures
  • Current licensure as a physical therapist in Ohio
  • CPR certification
  • Neurological Training and / or Certifications Preferred
  • Ability to thrive in a team environment including Physicians PTs and other rehab professionals.
  • Ability to educate patients families caregivers and patient treatment teams
  • Ability to manage a busy case load effectively.

Job Requirement

  • Graduate of accredited program in Physical Therapy.
  • Holds current license from the State of Ohio
  • Advanced degree related to a clinical specialty tract dual education and / or certification in neurological physical therapy preferred.(Not required)

  • Job Summary :

    Th e Physical Therapist completes initial assessments ongoing assessments and provides skilled therapeutic interventions to patients through the use of their educational knowledge skill and ability. This may involve outpatients inpatients pediatrics and offsite locations. Services the patients as a part of the entire continuum of professional care.

    Essential Functions :

    Eva luates patients gathering pertinent information from various sources and develops an appropriate treatment plan with specific goals. Responsible for ongoing assessment efficiency of treatment plan and revisions of plan as needed.

    Provides direct therapy to patients daily following patients plan of care and current standards of practice for Physical Therapy . Provides instruction in therapeutic procedures to patients and family.

    Maintains accurate and concise records on treatment provided and patients progress. Documents all care provided coordination of services orders goals etc. in the patients electronic medical record in an accurate and timely manner. Provides timely cosignature for supervised physical therapy assistants if applicable

    Maintains set level of productivity without jeopardizing quality. Delivers efficient and effective care to patients meeting and exceeding department standards.

    Maintains departmental policies and procedures regarding safety infection control time off requests and timecard recording. This will include all necessary education and yearly compliance requirements.

    Supervises the physical therapist assistants treatment interventions and professional growth.

    Education : None

    Required Licensing & Certifications :

    Licensed Physical Therapist in the state of current practice or meet all qualifications for traineeship or temporary license in the state of current practice (required)

    BLS Basic Life Support (preferred upon hire within 30 days for BSMH at hire for RSFH)

    Experience :

    2 years of experience in equivalent setting (preferred)

    Skills & Abilities :

    Demonstrates appropriate application of age specific standards policies and procedures and guidelines in caring for pediatric adolescent adult and geriatric patients

    Demonstrates general computer skills including data entry word processing email and records management.

    Analytical abilities to evaluate patient condition capabilities and progress.

    In terpersonal skills necessary to effectively interact with coworkers physicians patients and families.

    Requires the ability to work well with all levels of patients the public and other health care professionals.

    Demonstrates critical thinking skills.
