RN Clinical Manager Home Care

We are hiring an RN Clinical Manager for our Home Care agency in the Chilton, WI area to provide field and office duties, supervising clinical operations.

This position is Full time, on call rotation is required, some field visits and office time required.

The Registered Nurse in Home and Community- Based Services provides and directs provisions of nursing and personal care to patients in their homes as prescribed by the physician or as authorized by the state / programand in compliance with applicable laws, regulations, and agency policies. The Registered Nurse coordinates the planof care with other health care professionals involved in care and helps to achieve and maintain continuity of patient care by planning and exchanging information with physician, agency personnel, patient, family, case manager and other communityresources.

• Provides high quality clinical services within the scope of practice and within infection control standards, in accordance with the plan of care / service plan, and in coordination with other members of the patient / clients care team.
• Completes nursing assessments in accordance with federal and / or state program requirements and as required by payer. Ensures the patient / clients eligibility and medical necessity for services as defined by payer source and agency policy.
• Develops and revises individualized plans of care and / or service plans according to federal and / or state program requirements with other community provides. Provides oversight and supervision of the plan of care / service plan as required by agency policy, federal, and / or state program and as required by payer including services provided by the licensed practical nurse and paraprofessional, as assigned by supervisor.
• Ensures accurate and timely visit documentation of visits and services. Adheres to the electronic visit verification program as required. Reports clinical findings including changes in patient condition to the clinical manager / supervisor and physician timely.
• Performs treatments and intervention according to proper technique and procedure. Administers medications per physician order and in accordance with federal, state, or local regulations.
• Participates in and contributes to patient case conferences.
• Participates in the RN on-call rotation for the agency, if required.
• Completes all orientation and annual training timely as required by the organization and according to federal, state, or local regulations. Assists with the orientation of agency staff as needed.
• Participates on the organizations Performance Improvement and Compliance programs and adheres to policyand procedures.
• All other duties assigned.
