Serene Nursing Services is looking for skilled and experienced LPNs to join our growing team and work various shifts in Long term care facilities

This amazing LPN must :

• Provides direct patient care, assists with care for patients of others (as needed),
• Makes patient care assignments to other staff members on duty
• Responsible for the care delivered by the nursing staff under his / her direction during the shift for which (s)he is on duty.
• Communicates effectively all relevant information (e.g., patient information, safety issues, problems with equipment, quality issues, etc.) to immediate supervisor, in timely way.
• Communicates and documents changes in patient condition, on a timely basis, to charge nurse / supervisor and physician.
• Interprets, implements and enforces all facility policies and procedures
• Assigns responsibility for the direct care of residents
• Supervises the CNA staff
• Obtains and administers prescribed medications and treatments in accordance with approved nursing techniques.
• Observes patient, records significant conditions and reactions, and notifies resident family and physician informed of any changes in resident behavior, medications, or conditions of health.
