?? What You'll Do as Nurse Supervisor

Oversee and elevate the delivery of care for aging adults with complex needs to help them age in place

Conduct assessments, build care plans, and lead quality assurance initiatives so our clients thrive and we make an impact

Mentor caregivers to grow with confidence and compassion while developing their skills—teaching them a more holistic approach to care

Be a key point of contact for families, providers, and referral partners

Ensure seamless intake, transition planning, and ongoing care monitoring so that our patients get the best care and the best outcomes

Promote wellness using integrative care strategies …..not just task-based support—Focus on "the whole person"

Provide clinical support as part of an on-call rotation (evenings / weekends / holidays as needed)

?? Who You Are

A licensed RN or LVN with strong experience in home care, hospice, dementia care, or chronic conditions (but we would consider other past clinical positions)

Passionate about compassionate leadership and team development

Calm under pressure, grounded in empathy, and driven by integrity

Tech-comfortable and curious about innovation in healthcare

A strong communicator who listens well and leads by example

Someone looking for a long-term, stable role with a mission-driven, growing company

Based in or near San Francisco for in-person visits and team connection

?? $95,000 – $125,000 / year (based on license and experience)
