Review eligibility and complete enrollment referrals for members.
Coordinate care across healthcare providers, social services, and community resources.
Conduct assessments, create care plans, and provide education on treatments, medications, and follow-up needs.
Maintain accurate documentation in the electronic health record (EHR).
Regularly meet with members—primarily through home visits—to ensure continuity of care.
Advocate for members during medical visits when needed.
Identify and follow up on referrals to ensure member and family needs are met.
Reassess care plans to ensure they are effective and adjust as needed.
Respond to member questions, concerns, and requests promptly.
Recognize and report signs of child / elder abuse as required by law.
Support members with access to housing, transportation, food, and financial resources.
Qualifications
Must Have :
2+ years of experience in healthcare or a related field
Valid driver's license, clean driving record, and proof of insurance
Ability to travel locally within Placer County
Nice to Have :
Bilingual in English / Spanish
Experience with CalAIM, Enhanced Care Management (ECM), or similar programs
Skills & Attributes :
Strong communication skills (verbal and written)
Comfortable with computers, email, Microsoft Office, and virtual platforms
Problem-solving and critical-thinking abilities
Compassionate, reliable, and able to work independently
