School Social Worker - Wakefield, Rhode Island

Pay Rate : $50 / hr

Schedule : Full-Time, Onsite

Grades : 6th - 8th

Connected Health Care is seeking a dedicated and compassionate School Social Worker to join one of our trusted school partners in Wakefield, Rhode Island . This is a full-time, onsite position working with middle school students in grades 6-8.

The ideal candidate will be currently certified by the Rhode Island Department of Education (RIDE) as a School Social Worker and have a strong background in providing emotional, behavioral, and academic support to students within a school setting.

Key Responsibilities :

• Provide individual and group counseling to students to address academic, social, and emotional challenges.
• Serve as a key member of multidisciplinary teams, including the Evaluation Team, IEP Team, Crisis Intervention Team, and RTI / MTSS Team .
• Collaborate with teachers, administrators, and families to design and implement interventions that promote student success.
• Support the development and implementation of behavioral intervention plans.
• Participate in crisis response and provide social-emotional support to students and staff as needed.
• Maintain accurate documentation and ensure compliance with district and state guidelines.
