Compensation

Salary Range : $56.98 - $85.65 / hour

The pay range above represents the lowest possible rate for the position and the highest possible rate. Factors that may be used to determine where newly hired employees will be placed in the pay range include the employee specific skills and qualifications, relevant years of experience and comparison to other employees already in this role. Most often, a newly hired employee will be placed below the midpoint of the range. Pay for PRN positions is calculated differently than Full-Time, Regular Part-Time, and Part-Time positions. PRN Rates depend on the position and other factors, including years of experience, or shift commitment according to the department policy. PRN differential is added in certain positions.

Responsibilities

The Nurse Navigator enhances the patient experience by providing a seamless navigation process from abnormal finding / diagnosis though the cancer continuum. Responsible for collaborating with physicians and members of the interdisciplinary teams to triage, coordinate, and consistently manage patient care by educating the patient and serving as a point of contact for patients and families. Works with patients and families to answer questions, provide emotional support, and resolve any barriers, needs or issues related to the cycle of cancer care. Assesses patients' current and future needs, provides patient access to resources internal and external, facilitates distress screenings and patient needs assessments, and makes appropriate referrals. Establishes and maintains close collaboration with physicians and the cancer care team to ensure patient care and coordination using oncology nursing knowledge base, experience and expertise. Actively participates in multidisciplinary conference, providing patient follow up as needed. Provides patient education and symptom management support. Participates in community health promotion and awareness programs. Performs other duties as assigned.

Qualifications

Required :

California active RN license required.

• Associate of Science Degree in Nursing
• Minimum of Three (3) years of oncology nursing experience required
