General Description

Our Physicians serve our community by providing comprehensive direct medical care and services and working with other care team members to provide quality patient-centered care to our patients. Practiced specialists in our health centers included OB / GYN, Family Medicine, & Internal Medicine.

Salary determined based on qualifications and experience.

Essential Functions

• Works with staff, as a team, to provide high-quality care to our patients by identifying and assessing health concerns, examining the patient based on the health concern, and creating a plan specific to the patient needs.
• Develops and maintains an awareness of the Chronic Care Model Processes in order to reduce health disparities within our communities.
• Provides clinical consultation to other staff members and supervision of staff and students as assigned.
• Demonstrates adherence to National Patient Safety Goals and plan.
• Participates in quality improvement activities to assess, monitor and improve program services as well as mandatory in-services and drills, attends staff meetings and other training, including appropriate age-specific educational training offered on a regular basis for professional development.
