Key Responsibilities

Provide telephonic education and support to members with chronic illnesses

Conduct assessments and develop personalized health education plans for Medicare members

Collaborate with healthcare professionals to ensure members meet their treatment goals and access necessary resources

Required Qualifications

3+ years of related work experience in health education and chronic population management

Related Bachelor's degree in Health Education or a health-related field

Respiratory Therapist Certification

Experience working in a health plan setting or with telephonic health education preferred

CDCES Certification is preferred
