Job Summary and Qualifications

This position will be a combination of Case Management and Infection Prevention. Travel required for training, to be discussed upon interview.

Schedule : Monday-Friday, 8hr shifts, 36hrs a week

The Registered Nurse (RN) CM is responsible for promoting patient-centered care by coordinating the plan of care for the patient stay, managing the length of stay, ensuring appropriate resource management, and developing a safe appropriate discharge plan in collaboration with the multidisciplinary team. The RN CM facilitates the progression and transition of care using established criteria and in conjunction with the multidisciplinary team. The RN CM will coordinate activities that promote quality outcomes and patient throughput while supporting a balance of optimal care and appropriate resource utilization.

Highlands-Cashiers Hospital is a community hospital serving Macon, Jackson and the surrounding counties. Located between the towns of Highlands and Cashiers, the hospital offers 24 / 7 emergency care, acute inpatient care, rehabilitation, as well as long-term care through Eckerd Living Center. Ambulatory services are also available, including therapy services and primary care. Highlands-Cashiers Hospital has more than 24 beds for acute care services and 80+ beds in the Eckerd Living Center.
