Description
The Resident Care Coordinator is a working / Supervisory position responsible for the supervision of assisted living services : requires substantive interaction with residents, families, physicians, managers and staff from other departments; requires a high level of tact, persuasion, judgment, integrity and timing; coordinates resident services 24 hours a day while on duty.
RESPONSIBILITIES
- Provide and coordinate caregiver training as needed to comply with company policy.
- Performs Caregiver task as outlined in the resident personal service plan and scheduled by the Director of Clinical Care.
- Responsible for preparing and setting caregiver schedules to meet the needs of the community.
- Assists residents with activities of daily living ( ADL’s)-IADL’s
- Promptly answers emergency call systems, telephone and paging systems.
- Observe and report resident for changes in behavior, health or signs of abuse and notifies appropriate manager(s).
- Participates in all scheduled team member meetings pertaining to assisted living residents or staff as well as voluntary and mandatory on- going education and in-service training.
- Performs data entry in to Point Click Care program to update resident information, resident physical and emotional condition changes, and vitals, using appropriate terminology and abbreviations.
- Practice good infection control principles and uses appropriate hand washing techniques
- Maintain % compliance with current ADHS Rules and requirements.
- Coordinate resident care with Director of Clinical Care and caregivers.
- Oversees resident records, MARS and dispose of medication.
- Act as liaison between resident, family and primary care provider as necessary to maintain continuity of service. Conduct pre-move –in and on –going assessments.
- Performs and completes assignments within established procedures and guidelines
- Conduct assessments as required in the absence of the Director of Clinical Care.
- Enforce the importance of documenting Example : Incident, Fall Management and any change of condition etc.
- In absence of a Caregiver / Med performs duties as needed.
- Maintains and coordinates Caregiver Schedules to ensure resident service.
- Schedules Labs and X-rays as needed
- Miscellaneous duties as assigned by the Director of Clinical Care, Executive Director or Manager on Duty.

High School diploma or equivalentLicensed Certified Care Giver in accordance with Arizona State LawCurrent CPR and First Aide CertificatesCurrent Finger Print CardCurrent Food Handler’s CardAble to lift between 25 & 75 poundsExperience working with seniors and / or in the medical field (preferred)Ability to work in a team environmentPossess written and verbal skills for effective communication in EnglishMaintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment.Ability to work weekends, holidays and a flexible scheduleMust be able to function in a high stress work environment.SKILLS :
