LPN - Per Diem

The Daughters of Charity is a senior living community for the retired Sisters of the Daughters of Charity offering independent, assisted and memory care. We are located in Menands, NY, in a wonderful parklike setting. We are looking for a compassionate, team-oriented individual to join us as Per Diem LPN to work the 7am-3pm shift.

Our Nurse's Role Includes :

Assisting in the provision of the individualized, goal oriented, nursing care, utilizing the nursing process, in collaboration with the Director of Nursing and other members of the patient care team.

Establishing and maintaining a therapeutic relationship with each resident

Assist in determining current and potential needs and / or problems of Sister residents to create a care plan for each Sister based on individual needs.

Implement nursing actions according to the established care plan

Maintain professional discretion and strict confidentiality in all aspects of the position especially in regards to our Sister residents.

Assist in maintaining a clean, orderly and safe environment for our residents and staff

Maintain professional development through seminars, workshops and professional affiliations

Ability to react to change productively and handle other tasks as assigned

Participate in proactive team efforts to achieve departmental and community goals

Show leadership to others through example and sharing of knowledge / skills

Maintain infection control standards

Assist in evaluating some team members

Physical / Environmental Requirements

Must be able to physically and efficiently perform the duties of the position which involves the following : Standing and walking 2 / 3 of the shift, sitting 1 / 3 of the shift; must have use of hands and fingers to handle and feel; must be able to bend, stoop and reach; lifting may involve up to 300 lbs 1 / 3 to 2 / 3 of the day with the use of lifting equipment when needed.

Must have an unencumbered, valid NYS Nursing License as an LPN
