Position Overview
The Nurse Practitioner plays a key role in delivering high-quality clinical care to PACE participants. This position supports comprehensive assessment, treatment planning, care coordination, and ongoing health maintenance for seniors in a multidisciplinary, team-centered environment.
Responsibilities
Conduct health assessments and examinations for PACE applicants and participants.Serve as an active member of the Interdisciplinary Team (IDT).Evaluate participant needs and contribute to the development of comprehensive care and nursing treatment plans.Manage minor illnesses, injuries, and stable chronic conditions according to established procedures.Assess and respond to acute illnesses.Coordinate referrals with healthcare providers in consultation with the PACE physician.Conduct nursing home or home visits as directed.Provide clinical guidance and assistance to staff Registered Nurses.Participate in programs focused on health restoration, prevention, and maintenance.Attend and contribute to staff meetings and training programs.Participate in IDT meetings as a proxy when needed.Complete administrative duties such as charting and statistical records.Meet regularly with department directors and participate in in-service and supervisory training.Maintain professional competency through workshops and continuing education.Complete annual training in hazard communication, TB management, and bloodborne pathogen standards.Actively engage in Quality Assessment and Performance Improvement (QAPI) initiatives.Demonstrate excellence in age-specific care and participant communication.Promote participant rights, including dignity, independence, and confidentiality.Participate in community-focused activities supporting our mission.Meet productivity and customer service targets.Ensure nursing staff participate appropriately in fire safety and disaster preparedness drills.Experience & Qualifications
Licensure & Certifications :
Licensed RN & Nurse Practitioner in the State of CaliforniaMeets Board of Registered Nursing and California Business & Professions Code requirementsSuccessful completion of a Board-approved NP programCurrent CPR (BLS) certificationFirst Aid certificationTB screening and health examination required upon hireExperience :
Minimum one year of NP-level experience in a busy clinic; supervisory experience preferredAt least one year of experience working with frail or elderly populations in a paid position or volunteer roleSkills & Aptitudes :
Comprehensive Primary Care skillsPalliative and End-of-Life skillsCase Management skillsAbility to work effectively with the Interdisciplinary Team (IDT)Knowledge of PACE Regulations, CMS / state rules, Capitated Care modelBilingual Spanish or Korean is a plusJoin Our Mission
