Job descriptionManage a caseload of health care members with long-term care needs Conduct member assessments and document care plans Perform monthly and quarterly member contact (80% travel required) Complete in-person assessments with members, caregivers, or providers Monitor service delivery and conduct follow-ups via in-person or phone Coordinate referrals and ensure timely delivery of provider services Identify and address care plan deficiencies Collaborate with discharge planners and other health care professionals Assist members with complaints and appeals Bachelor’s degree in a relevant field 2+ years of medical case management experience Proficiency in Microsoft Office and electronic medical records Valid driver's license and reliable transportation High-speed internet and a quiet, secure home office Fieldwork / home visit experience Background in home health, discharge planning, or long-term care Bilingual in Spanish and English is a plus

- Pay Range : $22–$25 Hourly + Mileage Reimbursement
