Key Responsibilities :

🔹 Assess patients and develop individualized treatment plans to improve movement and function

🔹 Provide hands-on therapy and exercises to enhance strength, mobility, and balance

🔹 Educate patients and caregivers on rehabilitation techniques and injury prevention

🔹 Document patient progress and collaborate with interdisciplinary healthcare teams

🔹 Ensure compliance with facility policies and state / federal regulations

Qualifications :

• Active Physical Therapist (PT) license in CA
• Experience in skilled nursing, rehabilitation, or long-term care preferred
• Strong communication, patient-care, and teamwork skills
• Passion for helping patients achieve their highest level of independence
