Contract Type : 13-week contract

Guaranteed Hours : 37 hours per week

Shift : Monday - Friday, 8 : 00 AM - 4 : 00 PM (with some weekends and on-call required)

Pay Rate : $$ / hr (All-Inclusive)

Position Overview :

We are seeking an experienced Home Care and Hospice Registered Nurse (RN) to provide compassionate, high-quality care to patients in their homes and hospice settings. This role focuses on case management, direct patient care, and supporting families during end-of-life care, while ensuring compliance with State, Federal, and VNAHSR policies .

Key Responsibilities :

• Complete OASIS documentation accurately and on time, adhering to State, Federal, and agency guidelines.
• Assess patient and caregiver needs using the nursing process to provide safe, effective care.
• Develop and communicate individualized plans of care , involving patients and caregivers in decision-making.
• Serve as Case Manager , managing caseloads and visit schedules efficiently.
• Implement medical plans of care, nursing interventions, and procedures, including IV therapy, wound VAC, and complex wound care .
• Supervise and collaborate with LPNs and HCAs to ensure the care plan is followed.
• Educate patients and families on medications, treatment options, and discharge instructions .
• Identify and coordinate inter-disciplinary services, such as physical therapy and social work.
• Support safe patient discharges and transfers , ensuring all required paperwork is completed.
• Participate in case conferences , peer review audits, and performance improvement initiatives.
• Advocate for patients to ensure sensitive, compassionate, and high-quality care.
