Care Manager Primary Responsibilities :
Schedule caregivers with clients and communicate to all partiesConduct / train Article 9 and prevention and support classes to all employeesConducts daily / weekly reportingConducts quality calls and quality visitsConfirm and update caregiver availability including vacation, sick time, and holidaysVarifies payroll dailyEstablishes and build realtionships with contract providers to gain new clients and understand their needsRespond to employment leads regarding open positons with possible clientsDocument skills and attributes of caregivers to ensure strong matching to clientsFollow up with new caregiver hires to explain the scheduling processEstablish a great rapport with caregivers and clientsObtain ISP updates and enter HAB goals as per the ISPEnsure that authorization is in place prior to services startingIs on call as per on call calendar requirementsFollow up on all new leads, track needs and staff appropriatelyCare Manager Requirements :
Experience – One year experience in a home care setting preferred, and understand the DDD industry, and be willing to obtain Article 9 instructor certificationsEducation – Associates degree in any fieldSkills – Exceptional customer service skills, time management skills, excellent communication skills, willingness and able to become if not already an Article 9 and prevention and support instructor. Must be able and willing to train, mentor, interview and hire all new team members.Characteristics – This is an autonomous position, so we’re looking for individuals that are self-motivated and willing to learn.
