Manager of Case Management – Full-Time – Riverside, CA Area

Base Salary Range : $142,771.20 – $214,032.00 / year

Schedule : Full-Time | Monday – Friday | Admin Hours | Rotating Weekend On-Call

Relocation Assistance : Case by Case

An established acute care hospital in the Riverside, CA area is hiring an experienced Manager of Case Management to lead a multidisciplinary team of RN Case Managers and Assistants in a high-volume, high-acuity setting. The facility supports 500+ inpatient beds and a full spectrum of services across medical-surgical, critical care, behavioral health, and post-acute levels.

Job Description

Key Responsibilities :

Provide daily leadership and oversight to 15–18 Case Management FTEs

Manage throughput and discharge planning across all inpatient units

Lead rounds, escalate care barriers, and troubleshoot delays in progression

Coordinate with physicians, social services, and payers to ensure safe transitions of care

Supervise CM processes such as Interqual / M&R, insurance authorization, and placement

Ensure compliance with Medicare, Medi-Cal, commercial payer, and federal regulations

Deliver training, accountability, and mentorship to CM staff and assistants

Serve in a rotational on-call leadership role for LOA / HLOC / transport escalation

Qualifications

Bachelor’s Degree in Nursing (BSN) required

Current CA RN license (or pending)

At least 3–5 years of recent acute hospital case management experience

At least 1–2 years of experience in a leadership role in case management

Strong knowledge of Interqual, M&R, and payer-driven reimbursement models

Ability to engage with patients and families in discharge discussions with empathy and clarity

Proficient in identifying barriers to progression of care and resolving system-level issues

Ideal Candidate Profile :

Highly autonomous and able to hit the ground running with minimal onboarding

Skilled in staff coaching, throughput management, and interdepartmental collaboration

Familiar with leading in a union or high-volume acute care environment

Calm under pressure with strong critical thinking and service recovery skills
