Compensation : base salary, processed bi-weekly, plus bonus structure.

Shift Type :   Full-time, local travel, Monday-Friday.

Summary :

At LiveWell Partners, we’re looking for a compassionate and driven Clinical Liaison to support both our clinical coordination efforts and referral development strategy.

In this hybrid role, you will serve as the vital link between patients, hospitals, physicians, discharge planners, and our home health agency—ensuring that those in need of care experience a seamless transition to home. You'll also play a key role in developing referral partnerships that support agency growth.

Primary Responsibilities :

The Clinical Liaison is responsible for evaluating patients for home care appropriateness, initiating referral and admission processes, and fostering relationships with referral sources to drive growth. You will act as a Home Care Consultant, patient advocate, and strategic growth partner across your assigned territory.

Clinical & Care Coordination

Evaluate patients for home care appropriateness and recommend appropriate services.

Accept referrals, gather intake data, confirm insurance eligibility, and obtain physician orders.

Collaborate with internal staff to ensure smooth admission and start-of-care processes.

Educate patients and families about home care services, expectations, and available support.

Serve as a liaison between referral partners and agency staff to ensure timely, quality care delivery.

Ensure all referrals meet regulatory and agency criteria.

Sales & Referral Development

Develop and maintain relationships with physicians, discharge planners, facility staff, and community agencies to generate referrals.

Meet regularly with referral sources to discuss services, identify patient needs, and educate on eligibility and programs.

Conduct effective sales presentations and community outreach, including in-services, lunch-and-learns, health fairs, and CEU events.

Prepare and evaluate territory growth plans on a quarterly and annual basis.

Track referral trends, maintain customer profiles, and monitor competitor activity.

Maintain a weekly call plan and document all referral activity, follow-ups, and progress toward growth targets.

Meet or exceed referral targets within assigned territory.

Other Duties as assigned.

Qualifications :

Minimum of 3 years’ experience in the healthcare field (e.g., medical-surgical, home care, case management or discharge planning).

Sales experience in a healthcare setting a plus.

Active clinical licensure in state of practice.

Proficiency in Microsoft Office and CRM or referral tracking systems.

Must be highly organized, proactive, and comfortable managing multiple priorities.

Excellent verbal and written communication skills.

High degree of independence and accountability, with strong teamwork orientation.

Requirements :

Valid driver’s license and current auto liability insurance.

Must be able to work flexible hours to meet agency demands.
