What You’ll Do :

• 🩺 Direct Care & Case Management : Provide direct and supportive health care to individuals in residential programs, assessing current health status and identifying needs.
• 🏥 Health & Safety Oversight : Provide professional advice and direction on health and safety issues, ensuring the ongoing support of individuals’ health and medical needs.
• 🤝 Collaborate with Treatment Team : Establish positive working relationships with individuals, families, and interdisciplinary team members, both within and outside the agency.
• ⚠️ Emergency Response : Initiate nursing interventions in emergency and non-emergency situations, adhering to established policies and clinical standards of care.
• 📝 Documentation & Care Plans : Establish and maintain nursing care plans, document individual health needs, and follow up on required actions.
• 📅 Appointment Scheduling : Ensure timely scheduling of medical appointments and services for individuals in your caseload.
• 👩‍⚕️ Staff Training : Provide training to staff on nursing care plans and agency in-services, ensuring proper care delivery.
• 💊 Medication Management : Oversee medication administration, ensuring proper labeling, storage, and disposal of expired medications according to policy and regulations.
• 🏥 Hospitalization & Discharge Coordination : Communicate with hospital discharge planners and ensure safe discharges for individuals who have been hospitalized.
• 📑 Health Record Monitoring : Track and update health records, including seizures, bowel movements, vital signs, and other health metrics.
• 💼 After-Hours On-Call : Be available for after-hour on-call duties, responding to situations in accordance with policies and regulations.
• 🛡️ Safety Compliance : Follow safety guidelines to ensure a safe working environment and report any hazards.
• 🌍 Program Coverage : May be required to float or cover other programs as needed or adjust work schedules to meet the needs of individuals.
