WeCare Centers at Sugar Creek Care Center is looking for a motivated Director of Rehabilitation (DOR) for our community in Franklin, PA.  The primary purpose of the position is to supervise and maintain an efficient and professional rehabilitative program in accordance with our established policies and procedures to assure that the highest degree of quality care can be maintained at all times.

Key Responsibilities

• Plan, develop, organize, implement, evaluate, and direct rehabilitation services, as well as its programs and activities, in accordance with current rules, regulations, and guidelines that govern the long-term care facility.
• Meet with administration, medical and nursing staff, as well as other related departments in planning therapy services.
• Develop and maintain rehabilitation services standards.
• Evaluate and treat residents
• Participate in discharge planning, development and implementation of resident care plans, resident assessments, etc., as necessary.
• Assume the authority, responsibility and accountability of directing rehabilitation services.
• Ensure that residents are informed of consequences of not participating in prescribed therapy and document such action in the resident’s clinical record.
• Review complaints and grievances made by the resident, families, and / or visitors and make oral written reports to the administrator indicating what action(s) were taken to resolve the complaint or grievance.
• Ensure the resident participates in planning and scheduling of his or her treatment.
• Prepare and plan the rehabilitation services budget and submit to the Administrator for his / her review, recommendations, and / or approval.
