We are hiring a PRN Compassionate Licensed Medical Social Worker (LMSW or LBSW) to provide hospice social services in the Sherman area! If you are looking for a career with a purpose and a supportive team culture, then this is the position for you!

Our Licensed Medical Social Workers (LMSW) are responsible for the delivery of various social work services to hospice patients and their families. Our Social Workers (LMSW) provide social service counseling and guidance to facilitate the maximum functioning and coping capacity of each patient and caregiver / family member.

Responsibilities :

• Assesses the psychosocial status of patients and families / caregivers related to the patient's terminal illness and environment and communicates findings to the RN Case Manager and other members of staff including the interdisciplinary group.
• Assess the relationship of the patient's medical and nursing requirements to the patient's home situation, financial resources, and availability of community resources, including funeral arrangements if needed.
• Supervises the implementation of an individualized social work plan of care and notified the Bereavement Coordinator of bereavement assessment needs.
• Participates in the development of the individualized plan of care, involving the patient and family, and attends regularly scheduled interdisciplinary group meetings, assisting the team in recognizing the effects of the psychosocial stresses and symptoms of the terminal illness.
• Counsels patients and family / caregivers as needed regarding stress and other identified coping difficulties.
