About the Opportunity
Our client, a leading healthcare provider in Southern California, is seeking a dedicated Nurse Practitioner to provide high-quality care to high-risk patients in the San Fernando Valley region. This position offers the opportunity to make a meaningful impact by managing post-discharge care for patients with complex conditions, including Congestive Heart Failure (CHF) and Chronic Obstructive Pulmonary Disease (COPD), through home and clinic-based services.
Key Responsibilities
- Provide comprehensive care to high-risk patients in home and clinic settings.
- Follow evidence-based clinical guidelines to ensure quality patient care.
- Document all patient interactions and care provided in accordance with regulatory standards.
- Coordinate follow-up care, including access to social and behavioral health services.
- Monitor and document clinical and social issues for patients.
- Conduct all visits in a timely manner, adhering to group and health plan protocols.
- Perform additional duties as assigned.
Qualifications
Current certification as a Nurse Practitioner.Minimum of 3-4 years in healthcare, with specific experience managing COPD and CHF patients.Experience with high-risk, complex patient populations preferred.Home care experience is a plus.Excellent verbal and written communication skills in English (bilingual in Spanish preferred but not required).Proficiency in Microsoft Office applications.Ability to work independently while meeting team deadlines.Compensation and Benefits
