We are currently seeking a skilled and compassionate  Registered Nurse (RN) Hospice Case Manager  to join our team in the Lancaster, PA area .

Job Description :

• As an RN Hospice Case Manager, you will be responsible for managing and coordinating the care of patients in their homes, skilled nursing facilities, or assisted living environments.
• You will work closely with an interdisciplinary team to assess patient needs, develop care plans, and provide direct nursing care to ensure the comfort and dignity of patients at the end of life.
• This position requires a nurse with clinical expertise, compassion, and a commitment to providing high-quality hospice care.
