Position Overview
Seaford Center is seeking a motivated and skilled Occupational Therapist (OT) for a 14-week assignment. The OT will provide high-quality rehabilitative services focused on helping patients regain independence, enhance functional abilities, and improve their overall quality of life within a supportive post-acute care environment.
Key Responsibilities
- Conduct comprehensive patient evaluations and develop individualized treatment plans.
- Deliver evidence-based therapeutic interventions to improve ADLs, fine motor skills, strength, and functional mobility.
- Monitor patient progress and modify care plans as clinically appropriate.
- Accurately document all assessments, treatments, and patient responses according to facility standards.
- Collaborate with interdisciplinary teams-including PT, SLP, nursing, and case management-to support patient goals.
- Educate patients and families on therapeutic exercises, safety practices, and continued care strategies.
- Participate in care planning, discharge recommendations, and team meetings as needed.
- Maintain compliance with facility, state, and federal documentation and practice standards.
Requirements
Active state Occupational Therapist (OT) license.Graduate of an accredited OT program (OTD or MOT).Experience in post-acute, SNF, or rehabilitation settings preferred.Strong communication, clinical reasoning, and documentation abilities.Ability to work both independently and collaboratively within a multidisciplinary team.Current CPR / BLS certification.
