$41-47 / hr (DOE)

Phoenix, AZ

M-F 8 TO 4 : 30 PM

Position Summary : The Physical Therapist is responsible for providing and directing Physical Therapy services to guests,

including assessment, treatment, program planning and implementation, related documentation, and

communication and functions under physicians’ orders.

Education :

 Bachelor, Masters, or Doctorate of Physical Therapy from an accredited program.

 Successful completion of National Certification Examination for Registered Physical Therapist.

Licensure and / or Certifications :

 Current state licensure.
