Job Summary :
Location : Methodist Fremont Health
Address : 450 E 23rd St. - Fremont, NE
Work Schedule : Monday - Thursday 7 : 30 a.m. to 4 : 00 p.m. No nights, holidays, or weekends.
Would primarily serve as the Lung Cancer Clinic, Incidental Nodule & Lung Screening program navigator. Would also help support Hereditary Risk Clinic, Head and Neck Surgical Oncology, and Radiation Oncology as needed.
Responsible to manage Oncology patients and service across the continuum of health care services through collaboration with care givers, patients, and their family members, physicians and other health professionals to overcome healthcare system barriers.
Coordinates Oncology services to ensure achievement of patient outcomes in a timely manner, and enhances care giver, physicians, patient and family satisfaction across the continuum of health care services.
Promotes evidenced based medical practice of the patient population throughout the health care system.
Provides supervision of staff and patients involved in clinical oncology protocols / trials.
Responsibilities :
Essential Job Functions
1. Care planning to meet patient needs, notifying physicians of results, and maintain policies and procedures that meet regulations, but also promote innovation.
2. Patient care that addresses physical and psychosocial aspects in accordance with their needs and ensuring communication is affective throughout the treatment process.
3. Basic nursing competency assessment and response to emergency situations.
4. Supports the patients by coordinating resources to manage the disease process- including tools to assess and consult appropriate supports (i.e - Assesses distress, helping patients address issues, when unable consulting SW or other community partners).
5. Support the data and agenda for the Oncology Tumor Board and be an active participant to maintain certification for oncology.
6. Support the outpatient oncology clinics held at Methodist Fremont Health.
7. Supports the platform used to track genetically high risk patients.
