As a Care Coordination Manager at Frontera Health, you will play a vital role in ensuring that our clients receive timely and necessary care by managing our care coordination and scheduling functions. This role combines overseeing active and waitlist care coordination and scheduling across our Applied Behavior Analysis (ABA) therapy Clinics and Frontera Health Diagnostic services. The Care Coordination Manager role is essential for ensuring exceptional customer service for our clients and their families.
Key Responsibilities :
Lead and oversee care coordination for Clients from referral through graduation of ABA therapy, ensuring smooth and timely service delivery
Manage the scheduling functions for internal clinical employees and external clinical contractors including our :
Psychologists for Comprehensive Diagnostic Evaluations scheduling related to our waitlist and active Clients
Collaborate with team members to identify and analyze workflows for care coordination, optimizing efficiency and reducing errors
Lead a high-performing care coordination team responsible for supporting our Clients and Clinical team

We have determined a salary range for this position that takes into account several factors including experience, knowledge, education, skills, and abilities.
