Multiple opportunities available - Boston South; Metro Boston; Boston North; Western MA
Salary Range $125,000 to $135,000 a year
The Advanced Practice Nurse will be responsible for, but are not limited to :
- Conduct visits to patients on palliative care in accordance with local, state and federal regulations.
- Communicate visit findings to designated members of the interdisciplinary team, including the National Director of Palliative Care.
- Facilitate access to appropriate support models of care (i.e., hospice, rehabilitation, on-going aggressive treatment).
- Consult with other members of interdisciplinary team to contribute to the patient's plan of care.
- Prepare and maintain accurate patient records, charts and documents.
- Assist in development of clinical practice guidelines / standards in support of quality palliative care as needed.
- Ensure collaboration with patients, caregivers, medical directors and members of IDT.
Education :
Master's Degree in Nursing
Professional Experience :
Must have a Nurse Practitioner's license in the state of operationsMust have, at minimum, ONE year experience in the capacity as a Nurse Practitioner in a hospital, skilled nursing or acute care setting (or similar field)Board Certified in Internal Medicine, or General PracticeMust have a Master's DegreeMust have a valid driver's license and a reliable source of transportationMust be willing to travel locallyAbout Us
