The unit serves patients who require intermediate care and those downgraded from the CVICU. It heavily supports thoracic care, electrophysiology, catheterization lab procedures, and structural heart interventions. Common patient diagnoses include CVA, CHF, atrial fibrillation with rapid ventricular response, and other cardiac and vascular conditions. The nurse-to-patient ratio is generally 1 : 4, with 1 : 3 for post-surgical patients during the initial 24 hours. RNs are required to titrate various drips, including amiodarone, nitroglycerin, nicardipine, epinephrine, and diltiazem for blood pressure management. There will be a charge nurse on each shift, who may rarely take an assignment. Patient care techs will be available, with a ratio of 1 : 18 for patient assistance. Hospitalists and intensivists are in-house 24 / 7, with cardiology on call after hours. Respiratory therapy is also available in the unit around the clock. RNs will need to read and interpret telemetry strips, and remote telemetry techs will provide additional support. Shifts are scheduled from 0630–1900 and 1830–0700, with a requirement for weekend rotations every other week. Floating to all same-level and lower acuity units may be required. The dress code consists of navy blue scrubs. Orientation will be tailored based on individual experience and needs. Additional time off requests should be communicated upfront, as additional time off will not be approved after submission. The hospital reserves the right to cancel up to 4 shifts during a 13-week assignment.
