Job Description

Administer skilled nursing care to patients in their place of residence as prescribed by the physician and according to the established plan of care.

Work under the supervision of a Registered Nurse (RN) and coordinate care with the interdisciplinary team and the patient / family.

Deliver care in compliance with State, Federal, and Medicare standards.

Assist the RN in providing nursing services and identifying the patient's physical, psycho-social, and environmental needs.

Participate in planning, coordinating, and implementing care in conjunction with the RN.

Provide effective communication to patient / family, co-workers, and other health care professionals.

Assist the patient / family in learning appropriate self-care techniques.

Provide care utilizing infection control measures that protect both staff and patient.

Assist the RN and / or physician in performing specialized duties related to end-of-life care for Hospice patients.

Assist with admitting, discharging, and transferring of patients.

Possible on-call duties.

Participate in annual in-service and performance improvement programs.

Perform other related duties as assigned.

Critical Information

Current Nursing License in the Practicing State.

Valid Driver's License and current auto liability insurance coverage.

Ability to recognize and maintain professional boundaries in relationships.

Ability to recognize an unsafe working environment and notify a supervisor immediately.

Must read, write, and speak fluent English.

Must have good and regular attendance.

Education / Licenses Needed

Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.

Current CPR Certification.

Two years of experience as an LPN / LVN in a clinical setting, preferably in a home health or hospice setting.

Benefits :
