Registered Nurse (Per Diem) - O.R.

Shifts : 7a-330p, 3p-11p,11p-7a,9a-630p flexible

PRN rate - $60 / hr.

Summary :

To provide direct and indirect nursing care to a patient or a group of patients.

Responsibilities :

1. Physical Assessment Criteria A : Completes the Preoperative assessment of the patient providing a clear picture of his / her status. Documents findings on the patient memorandum. Physical Assessment Criteria B : Documents detailed information incorporating the plan of care, interventions and outcomes reflecting the patients physical, mental and emotional status. Submits audits of their documentation from 3 different charts annually. Physical Assessment Criteria C : Continually reassess the patient during the operative procedure. Documents any changes / interventions as it relates to the patients clinical status. Physical Assessment Criteria D : Completes patients memorandum and other pertinent documentation including an assessment of the patients needs to determine the preferred level of care once transfers to the PACU, AMBI, or ICU.

2. Administration of Medications Criteria A : Follows the 5 Rights of Medication Administration, verbalizing to anesthesia, CST, RN, PA and Surgeons medication being dispensed onto the sterile field. Administration of Medications Criteria B : Assesses the patient prior to administering medications noting allergies, parameters, potential side effects, effectiveness and intervening as needed. Administration of Medications Criteria C : Provides pain management support to anesthesia and the surgeon based on the patients reports and assessment findings. Reassesses and documents response according to hospital standards. Administration of Medications Criteria D : Identifies high risk : PINCH, sound alike / look alike and utilizes medication references as needed.

3. Patient Safety Criteria A : Assesses patients for potential safety risks upon admission to include consent and site verification protocol. Patient Safety Criteria B : Adheres to hospital policies and procedures regarding patient safety, communicating potential safety risks from relief to relief, upon transfer and when there is a change in condition. Patient Safety Criteria C : Initiates interventions related to the risk factors identified. Patient Safety Criteria D : Conducts a Surgical "Time Out" per hospital protocol engaging all surgical team members. Confirm that all surgical team members are in agreement prior to incision.

4. Patient / Family Education Criteria A : Assesses learning needs of patient / family / significant other prior to surgery, answer any questions and or concerns to decrease stressors. Patient / Family Education Criteria B : Plans and provides appropriate instruction using applicable teaching methods. Explains Operating room routine and an overview of the procedure. Patient / Family Education Criteria C : Assesses the patient / family / significant other response to teaching and reviews instructions. Patient / Family Education Criteria D : Reports significant findings to the Patient Care Manager and or the Director of Surgical Services.

5. Delegation Criteria A : Applies the 5 rights of delegation. Delegation Criteria B : Coordinates and supervises the delivery of nursing care, including appropriate delegation of tasks. Delegation Criteria C : Maintains accountability for care delegated. Delegation Criteria D : States roles / responsibilities of co-workers to whom tasks are delegated.

6. Leadership Criteria A : Promotes a positive work environment through personal example and teamwork. Leadership Criteria B : Assumes role of charge nurse demonstrating independent problem solving, decision making, organizational and prioritization skills. Leadership Criteria C : Conducts walking rounds in order to attends to unit staffing needs in relation to acuity and census in a pro-active manner. Leadership Criteria D : Assists in orientation and teaching of new nursing personnel.

7. Skin Care Criteria A : Assesses patients skin condition preoperatively and post operatively documenting any change in condition prior to transfer. Institutes protocols per Norwalk Hospital policies based on assessment. Skin Care Criteria B : Initiates preventive skin care measures for all patients by padding any and all bony prominences. For those patients at high risk for skin breakdown, utilizes the pressure mattress in addition to using padding. Skin Care Criteria C : Positions the patient while still awake when possible to decreases chances of injury to skin or patients mobility. Skin Care Criteria D : Maintain skin integrity during the patient's operative course and adjust the plan of care according to outcomes.

8. Care Delivery Criteria A : Adheres to Nursing Standards of Practice to include assessment, care planning, interventions and evaluation of the plan of care. Care Delivery Criteria B : Practices according to unit-based standards. (see unit specific competencies) Care Delivery Criteria C : Conducts and participates in shift report for continuum of patient care delivery when either being temporarily or permanently relieved taking into account needle, sharps and sponge counts. Care Delivery Criteria D : Maintains and controls the intraoperative environment for maximizing optimal patient care outcomes.

9. Performs other duties as assigned.

10. Fulfills all compliance responsibilities related to the position.

Education : ASSOCIATE'S LVL DGRE

Other Information :

Required : License to practice in the State of Connecticut.

Minimum Experience : one year

Desired : Basic Cardiac Life Support (CPR), Trauma hours, as per unit standards

Working Conditions :

Manual : significant manual skills / motor coord & finger dexterity

Occupational : Significant occupational risk

Physical Effort : Very Heavy effort. May exert up to 50 lbs. force

Physical Environment : Significant exposure to dirt, odors, noise, human waste, etc.
