This LVN position offers the opportunity to deliver in-home care to patients in various locations across Los Angeles and West Los Angeles, including :

Culver City, Los Angeles, El Segundo, Hawthorne, Santa Monica, Echo Park, Mar Vista, Inglewood, Downtown, Mid-Wilshire, South Los Angeles and other surrounding areas in Greater Los Angeles

The ideal Licensed Vocational Nurse (LVN) is passionate about delivering superior, patient-centered care and is an excellent communicator with all types of patients. Under the direction of the Director of Patient Care, the Licensed Vocational Nurse (LVN) provides services to patients in accordance with the State Nurse Practice Act, agency policies, and accepted professional standards of practice utilizing a comprehensive base of nursing theory and nursing process and communicates and documents observations and assessments.

What you will need to accomplish :

At GrandCare Health Services, we expect LVNs to deliver high-quality patient care by performing the following responsibilities :

• Providing clear and accurate health care instructions to patients, based on thorough assessments and care plans.
• Effectively implementing physician orders, ensuring proper care delivery.
• Promptly informing the Nurse Case Manager, Director of Patient Services, or Clinical Supervisor of any significant changes in the patient’s condition.
• Completing detailed and timely documentation using the company’s Electronic Medical Record (EMR) system via company-issued tablet.
• Engaging both patients and their families in the care planning process, fostering collaboration and communication.
• Initiating appropriate preventative and rehabilitative clinical procedures to support patient recovery and well-being.
• Educating patients and their families on how to address clinical needs and related care requirements.
• Maintaining patient privacy, ensuring a safe home environment, and managing medication safety effectively.
