Gurwin Jewish Nursing & Rehabilitation Center is a renowned healthcare provider located on Long Island, NY which offers a full continuum of services. The Nursing & Rehabilitation Center is a 460 bed facility providing services for short-term rehabilitation and long-term skilled nursing care, home care programs, an adult day health program, ventilator dependent care, palliative and hospice care, a new Memory Care Unit and on-site dialysis. The Gurwin Family of Services also includes the 201-apartment Gurwin Jewish ~ Fay J. Lindner assisted living community and the Fountaingate Gardens independent living community on the Commack campus.
We currently have a career opportunity for a Staff Nurse, R.N. - Admissions for our evening shift who would exemplify the passion and commitment that is required to deliver the ultimate care for our residents in regard to doing admission assessments and all components for new admissions / re-admissions.

Responsibilities include, but are not limited to :
Assessment, Orders / Pick-up, Care Plan – Initiate, Risk Assessments, any other nursing component that has to
do with facilitating the Admission process
Assumes responsibilities for the formulation and implementation of patient care plans to meet the specialized needs of geriatric patients using nursing assessment and diagnostic skills.
As a unit situation requires, if there are no admissions the Admissions Nurse can be utilized to do observations, pick up orders and do risk assessments. The Admissions Nurse may be assigned to assume team leader, primary nurse, or charge nurse responsibilities. Administers direct patient care as indicated by assessment of patient condition and unit need.
Assists in direction, supervision, instruction, and evaluation of other nursing personnel as assigned.
Observes, records, and reports patient data both verbally and in writing as dictated by departmental policy.
Participates in unit nursing and inter-disciplinary conferences. Assumes a role in planning unit management with the Assistant Head Nurse and Head Nurse.
Assumes responsibility for adherence and support of the institutional and departmental philosophy and policy in professional practice.
Attends nursing and other institutional in-service programs and participates in departmental and institutional committees as assigned.
Performs related and miscellaneous duties as directed. In an emergency situation, may be expected to perform unrelated duties.
Ability to respond flexibly to a dynamic clinical environment.
