Position Overview :

As a Home Health Occupational Therapist , you will :

  • Evaluate Patients : Assess patients' physical, cognitive, and functional abilities to develop individualized treatment plans.
  • Develop Treatment Plans : Create and implement therapy programs aimed at improving daily living and work skills.
  • Provide Therapeutic Interventions : Utilize evidence-based techniques to enhance patients' independence and quality of life.
  • Educate Patients and Families : Offer guidance on adaptive equipment and home modifications to ensure a safe environment.
  • Collaborate with Healthcare Team : Work closely with physicians, nurses, and other therapists to coordinate comprehensive care.
  • Document Progress : Maintain accurate records of patient evaluations, treatment plans, and outcomes.

What We're Looking For :

  • Licensed Professional : Active license and registration New York State.
  • Experience : experience in occupational therapy; home health experience is preferred.
  • Communication Skills : Excellent interpersonal skills to effectively interact with patients, families, and team members.
  • Reliability : Strong organizational skills and the ability to work independently in a home setting.
