Job Description :

• The unit is comprised of 24 acute / swing beds, 40 nursing home beds, and 10 skilled beds within the nursing home.
• Patient ratio is maintained at 1 : 1.
• The rehab department includes PT, OT, and SLP professionals, supported by unit secretaries and tech staff.
• Treatments are provided in patients' rooms, hallways, outdoor areas, and in the rehab gym.
• Use of documentation systems Cerner and NetHealth is required.
• Schedule requires 40 hours per week with specifics reviewed during an interview with the rehab manager.
• Clinicians may need to float between acute / swing, nursing home, and outpatient services as required.
• Procedures include physical therapy evaluations, establishing plans of care, assisting with rounds and care conferences, and regular communication with the rehab team regarding patient progress, equipment needs, and discharge requirements.
• Dress code is business casual or scrubs.
