Registered Nurse In Personal Care Services
The Registered Nurse in Personal Care Services provides and directs provisions of nursing and personal care to patients in their homes as prescribed by the physician or as authorized by the state / program and in compliance with applicable laws, regulations, and agency policies. The Registered Nurse coordinates the plan of care with other health care professionals involved in care and helps to achieve and maintain continuity of patient care by planning and exchanging information with physician, agency personnel, patient, family, case manager and other community resources.
Responsibilities include :
- Providing high quality clinical services within the scope of practice and within infection control standards, in accordance with the plan of care / service plan, and in coordination with other members of the patient / client's care team.
- Completing nursing assessments in accordance with federal and / or state program requirements and as required by payer. Ensures the patient / client's eligibility and medical necessity for services as defined by payer source and agency policy.
- Developing and revising individualized plans of care and / or service plans according to federal and / or state program requirements with other community providers. Provides oversight and supervision of the plan of care / service plan as required by agency policy, federal, and / or state program and as required by payer including services provided by the licensed practical nurse and paraprofessional, as assigned by supervisor.
- Ensuring accurate and timely visit documentation of visits and services. Adheres to the electronic visit verification program as required.
- Reporting clinical findings including changes in patient condition to the clinical manager / supervisor and physician timely.
- Performing treatments and intervention according to proper technique and procedure. Administering medications per physician order and in accordance with federal, state, or local regulations.
- Participating in and contributing to patient case conferences.
- Participating in the RN on-call rotation for the agency, if required.
- Completing all orientation and annual training timely as required by the organization and according to federal, state, or local regulations. Assisting with the orientation of agency staff as needed.
- Participating on the organization's Performance Improvement and Compliance programs and adhering to policy and procedures.
- All other duties assigned.
Education and Experience Desired :
